In short

Dentin is the layer beneath the enamel, softer and roughly 70 percent mineral, and it makes up the bulk of the tooth. It is riddled with microscopic tubules containing fluid and cell processes connected to the pulp, which is why exposed dentin produces sensitivity. Unlike enamel, dentin has a limited capacity to respond and lay down new protective layers.

The tubules explain almost everything

Dentin is not a solid block. It is perforated by tens of thousands of tubules per square millimetre, each running from the pulp outward and containing fluid plus the process of an odontoblast cell. When exposed dentin meets cold, heat, air, sweetness, or touch, the fluid in those tubules moves, and that movement stimulates nerve endings at the pulp end. This hydrodynamic mechanism is why sensitivity is sharp, immediate, and stops when the stimulus is removed.

It also explains why sensitivity is not a defect but a signal. Dentin is designed to be covered — by enamel above the gumline and by cementum and gum below it. Sensitivity means that covering has been breached somewhere: worn enamel, receded gum, an abrasion notch, a crack, decay, or a leaking restoration margin. The productive question is never only "how do I stop the pain" but "where is dentin exposed and why".

This capacity is the biological basis for conservative treatment of deep cavities. Rather than removing every trace of affected dentin and risking pulp exposure, a dentist may leave a thin layer, seal it well, and allow the pulp to lay down reparative dentin underneath — a technique that saves nerves which older approaches would have sacrificed.

Common causes of dentin exposure

  • Gum recession exposing root surface, which has no enamel covering at all and is therefore immediately sensitive. Frequently follows periodontal disease or over-vigorous brushing.
  • Abrasion notches at the gumline from hard brushing with abrasive paste.
  • Erosion from dietary or gastric acid thinning the enamel.
  • Attrition from grinding, wearing through enamel on the biting surfaces.
  • Decay penetrating enamel.
  • Chips and fractures exposing a dentin surface directly.
  • After whitening or a cleaning — usually temporary, as peroxide passes through the tubules and scaling exposes previously covered root surfaces.

Managing sensitivity, in a sensible order

  1. Get the cause identified. Sensitivity from a receded gum, from a crack, and from decay feel similar to the patient but need entirely different treatment. Persistent sensitivity in one specific tooth is more concerning than generalised sensitivity across many.
  2. Desensitising toothpaste with potassium nitrate or stannous fluoride, applied by rubbing a small amount directly onto the sensitive area with a fingertip and leaving it rather than rinsing immediately. Expect two to four weeks for meaningful effect, not two days.
  3. Stop the mechanical cause — soft brush, light pressure, no abrasive whitening paste on the affected area.
  4. Reduce acid exposure and never brush within thirty minutes of it.
  5. In-office fluoride varnish or sealant over exposed areas.
  6. Restore abrasion notches, decay, or fractures where present.
  7. Splint for grinding if attrition is the mechanism.
Sensitivity that changes character needs examination, not stronger toothpaste. The reassuring pattern is a sharp pain that stops as soon as the stimulus is removed. Warning signs are pain that lingers for minutes afterwards, pain that wakes you at night, or pain that arrives spontaneously with no trigger — these suggest the pulp itself is inflamed as pulpitis rather than dentin merely being exposed, and that is a different problem with a different urgency.

Why teeth yellow with age

Dentin is naturally yellower than enamel, and two changes work together over decades: enamel gradually thins and becomes more translucent, while the pulp continues depositing secondary dentin and thickens that inner layer. The result is progressively more yellow showing through progressively less white. It is anatomy, not neglect — which also explains why bleaching has limits, since it acts on the tooth's mineral content and cannot restore lost enamel thickness. Why are my teeth sensitive to cold?

Does desensitising toothpaste actually work?

Yes, for genuine dentin sensitivity, but it needs correct use and realistic timing. Rub a small amount directly onto the sensitive area with a fingertip and leave it rather than rinsing straight away, and allow two to four weeks for meaningful effect. Most people who conclude it does not work either rinsed it off immediately or gave it only a few days.

Can dentin repair itself?

To a limited but real extent, unlike enamel. Tubules can mineralise and narrow, reducing sensitivity over time, and the pulp can lay down reparative dentin beneath an injury. This capacity is why a dentist may deliberately leave a thin layer of affected dentin in a deep cavity and seal over it, allowing the tooth to build its own barrier and preserving the nerve.

Why did my teeth become sensitive after a cleaning?

Because scaling removes calculus that was covering root surfaces, exposing dentin that had been shielded. It is usually temporary and settles over days to a couple of weeks as the tubules mineralise. Using a desensitising paste during that period helps. It is worth mentioning if it persists beyond a few weeks, since that suggests another cause.

When is sensitivity a sign of something more serious?

When the character changes. Sharp pain that stops the moment the stimulus is removed is typical of exposed dentin. Pain that lingers for minutes, wakes you at night, or arises spontaneously without any trigger suggests the pulp itself is inflamed, which is a different diagnosis with greater urgency. Sensitivity localised to one tooth also deserves examination rather than toothpaste.
