First aligner fitting and attachment placement

We fit the first aligner and place attachments on the tooth surfaces. To bond them precisely where ClinCheck planned, we use an attachment template. IPR (interproximal reduction; minor enamel reduction between teeth) is also performed at this visit.

Regular checkups (every 6~8 weeks)

Retainer placement and maintenance

After treatment, you’ll receive 2~4 sets of Vivera retainers (made by Invisalign). 09 Attachments and IPR

What are attachments?

Attachments are small composite resin bumps bonded to the tooth surface that let Invisalign function as a precise orthodontic appliance rather than just a 'clear mouthpiece.' On average, 8~15 attachments are placed per person.

Attachment placement·removal

Attachments are bonded using light-cured composite resin (tooth-colored). There is no pain during placement, and it takes 1~2 hours. At the end of treatment, we remove them painlessly with a diamond bur and polish the surface to restore a natural tooth finish. Any effect on the enamel surface is minimal at 0.05mm or less.

What is IPR (Interproximal Reduction)?

IPR is a procedure that creates space by subtly reducing 0.1~0.5mm of enamel on the sides (interproximal surfaces) of the teeth. It is also called "interproximal reduction" or "stripping."

Why is IPR needed?

Safety of IPR

Enamel thickness averages 1~2mm, and IPR removes about 0.3~0.5mm per side. Thus only 30~50% of the enamel thickness is reduced, and the risk of cavities is very low (even safer with fluoride application). Over 30 years of clinical data have reported little to no caries, sensitivity, or root problems caused by IPR.

IPR vs Extraction

If similar space can be gained, IPR is far more conservative than extraction. Being able to gain 6mm of space without removing a single tooth makes IPR one of Invisalign’s major advantages. However, severe protrusion or significant malocclusion that truly require 4 premolar extractions cannot be replaced by IPR.

Important side effects (need prevention and management)

1. Cavities around attachments

Food can easily get trapped between attachments and teeth, and saliva can become more acidic under the aligner, increasing cavity risk. Brush after every meal and use a fluoride rinse before bed. Average cavity incidence is about 5~10%, and with good hygiene it can be 1% or less.

2. Root resorption

A complication that can occur with any orthodontic treatment; in Invisalign, mild root resorption occurs in about 5% of patients (average 0.5~1mm). The incidence is similar to, or slightly lower than, wire braces. Monitor with regular X-rays, and consider a temporary pause in treatment if resorption progresses.

3. Gum recession

If teeth are moved outside the bony housing, the gums may not follow and recession can occur. Incidence is about 3~7%, and it is more common in young to middle-aged smokers and in people with thin gums. At the ClinCheck stage, design movements to keep root positions within the bone to prevent this.

4. TMJ (Jaw Joint) symptoms

During treatment, temporary TMJ discomfort (clicking, pressure) can appear in about 10~15%. Most resolve after treatment is finished, but patients with preexisting TMJ issues may worsen, so pre-treatment evaluation is important.

5. Allergic reactions (very rare)

Allergic reactions to the SmartTrack material are very rare at under 0.01%, but if suspected, stop immediately. If you have a history of polyurethane allergy, a pre-treatment patch test is available.

"Diamond = always good" is not necessarily true

Diamond level is only a quantitative indicator of case volume; it is not an absolute guarantee of skill. Consider a combination of factors—provider tier, the doctor’s education and experience, ClinCheck explanation ability, and their regular follow-up system. All else being equal, higher-tier providers are statistically more likely to deliver better outcomes.

Do I need to prepare anything before starting Invisalign?
Before starting orthodontic treatment, please complete: ① treatment for cavities and gum disease, ② extraction of impacted wisdom teeth (if needed), ③ evaluation of aging restorations (crowns), ④ Scaling (Dental Cleaning). An average pre-treatment period of 1~3 months is required.
What if I become pregnant during Invisalign treatment?
Invisalign during pregnancy is generally safe. SmartTrack material is BPA-free and FDA Class II certified. However, due to early-pregnancy gum changes, a temporary pause may be recommended. Because X-ray exams are limited during pregnancy, regular check-ups can be restricted; please decide after consulting your OB/GYN.
What if I get a cavity during Invisalign?
It depends on where the cavity is. If it’s small and not in an area that affects attachments, we can treat it while you keep wearing your aligners. If it’s a large cavity that needs a Crown, we pause treatment to make the Crown, then restart with a new ClinCheck.
I lost my aligner!
Contact the clinic immediately. ① If you have the next set, move to it right away. ② If you still have the previous set, wear the previous set temporarily. ③ If you have neither, place a rush order for a new set (1~2 weeks). Going without aligners for more than 24 hours can let teeth start shifting.
I cracked my aligner.
It depends on where it broke. ① Minor edge crack — you can keep using it. ② Crack in the middle — switch early to the next set. ③ Complete break — rush-order a new set. In any case, please inform the clinic.
Can I drink an Americano during Invisalign?
Remove your aligners before drinking. Dark beverages like coffee, black tea, and cola can stain aligners, and hot drinks can warp them. After drinking, brush or rinse before putting them back in.
It hurts when I sleep with the aligners in.
For the first 2~3 days after switching to a new aligner, pressure or soreness is common. Short-term use of a pain reliever like ibuprofen can help. If it lasts over 1 week or is severe, contact the clinic.
My aligner changed color.
Common causes of discoloration are ① dark beverages, ② smoking, ③ insufficient cleaning. Soak in a cleanser (Retainer Brite, Polident) and brush gently; in most cases it clears up. If not, switch early to the next set.
Can I get other dental care during Invisalign treatment?
It depends on the procedure. Scaling (Dental Cleaning) and simple cavity treatment are fine. But for major procedures like a Crown, extraction, Dental Implant, or Root Canal Treatment, consult your Invisalign provider in advance.
A Wisdom Tooth started coming in during Invisalign.
Wisdom Tooth eruption during orthodontic treatment can push other teeth and interfere with alignment. Extraction is often recommended; if it’s impacted, a surgical extraction may be needed. This can extend treatment time by 1~2 months.
How should I pack my aligners for a trip?
① Bring your current aligners + the next 1~2 sets. ② Carry the storage case (to prevent loss). ③ Pack a travel kit with toothbrush, toothpaste, and floss. ④ Keep your clinic’s emergency contact info handy. For long trips, also pack the previous set as a backup.
I have a wedding during Invisalign.
On the wedding day, you may take your aligners out (up to 24 hours; longer is risky). Attachments are slightly visible but tooth-colored, so they’re barely noticeable from a distance. If you’d like them temporarily removed for photos, discuss it with your provider. Usually, temporary removal 1~2 weeks before the wedding → re-bonding after the event is possible.
My teeth shifted again after finishing Invisalign.
This often happens if retainers aren’t worn as directed. Depending on how much relapse there is: ① Mild — re-align with Vivera Express (5~7 aligners). ② Moderate — a restart with Lite or Moderate may be needed. Acting quickly helps minimize cost.

Can I fly during Invisalign?
Totally safe. Cabin pressure changes don’t affect aligners. On long flights, carry your case, a mini toothpaste, and the next set.
I’m older—can I still do Invisalign?
If your gums and tooth roots are healthy, it’s possible even in the 70s~80s. However, if bone loss is over 50%, it’s not recommended. Decide after a periodontal evaluation. In older adults, tooth movement is 30~40% slower, so treatment takes longer.
Can I still have double-jaw (orthognathic) surgery after Invisalign?
Yes. The sequence is: pre-surgical orthodontics (to align teeth) → double-jaw (orthognathic) surgery → post-surgical finishing. Choose a Diamond-tier provider with extensive experience in orthognathic cases.
Wearing the aligners feels too uncomfortable.
Most people adapt within 1~2 weeks. Try: ① Sleep a bit more the first few days to speed adjustment. ② Practice speaking (read aloud). ③ Sip water often to ease dry mouth. If you’re not adapting after over 1 month, consult your provider.
I produce too much saliva when I wear the aligners.

Can I kiss while wearing Invisalign aligners?

Can I exercise while on Invisalign?
In most cases, yes. Gym/weight training, running, and cycling are fine. For contact sports such as martial arts, rugby, and hockey, use a separate mouthguard (aligners are not for impact absorption).
My aligner isn’t seating properly
It’s normal for a new aligner to feel tight for the first 1~2 days. ① Press down slowly and evenly with both hands, ② Use "Chewies" — silicone pads for seating aligners, ③ Do not soak in warm water before inserting (risk of warping). Can I drink alcohol during Invisalign?

Is it true you may lose weight during Invisalign?

I have a Dental Implant — can I get Invisalign?
Yes. However, a Dental Implant is firmly anchored to bone and cannot be moved. ClinCheck planning should move only the natural teeth and keep the implant in place.
I have a Crown/Dental Bridge — can I get Invisalign?
Yes. However, attachments may not bond well to a crown surface, so positioning may need adjustment. The crown may need to be remade after orthodontic treatment (especially if tooth position changes significantly).
I’m getting sores on my tongue
Common during the first 1~2 weeks of adaptation. ① Smooth rough edges with dental wax, ② Rinse with warm salt water, ③ Your dentist can trim the aligner edge. If it continues, please contact our dental team.
I’m experiencing tooth sensitivity during Invisalign
Mild sensitivity during treatment is common. As teeth move, the nerve can be temporarily more sensitive and usually recovers within 1~2 months. Use a desensitizing toothpaste (e.g., Sensodyne). Ongoing or severe sensitivity may indicate a cavity or nerve issue—please come in for an exam.
What happens if I don’t wear my retainer?
Within 1 year after finishing, not wearing your retainer leads to visible relapse in 30% or more of patients (especially the lower front teeth). If relapse occurs: ① Mild — quick re-alignment with Express is possible, ② Severe — restarting with Lite/Moderate is needed. Wearing your retainer every night is the most affordable lifelong insurance.
Do I need to have my Wisdom Teeth removed after Invisalign?
In most cases, yes. When Wisdom Teeth erupt, they can push the front teeth and are a leading cause of relapse. If they are impacted, we recommend extraction within 1~3 years after finishing orthodontic treatment.
Will my Invisalign results look as good as the photos?
Tooth alignment and bite usually match the ClinCheck simulation closely (85~90% with Diamond-level providers). However, gumline and facial changes vary by case, so use your ClinCheck outcome as a realistic expectation. The final 5% of fine-tuning is where Diamond-level providers make the difference.
Can a Dental Implant be placed during Invisalign treatment?
Yes. We reflect the placement site and timing in ClinCheck. ① Most commonly, the implant is placed after orthodontic treatment, ② If an implant is needed during treatment, place it first and lock that position in ClinCheck. Choose a clinic that offers multidisciplinary care.
Will starting Invisalign change my face shape?
